Anti-CMV antibodies and coronary artery disease.

Artery Disease To the Editor: Recently, Ridker et al published their observations about cytomegalovirus (CMV) and atherosclerosis.1 They found that “there is no evidence of a positive association between baseline IgG antibodies directed against CMV or HSV [herpes simplex virus] and the development of future MI [myocardial infarction] or thromboembolic stroke. Furthermore, we found no evidence of association between CMV or HSV exposure and concentration of CRP [C-reactive protein]”.1 I would like to mention 2 important factors that were mentioned in the article but were not evaluated carefully: 1. The IgG level was measured only at study entry, and the follow-up continued for 12 years. It is true that an antibody titer (especially IgG) does not change “quickly”; however, it is a little extreme to assume that an antibody titer does not change over 12 years! 2. I would like to refer the authors to our study that was published recently.2 In this study, we wrote that “the results of our study demonstrated a similar prevalence of CMV seropositivity in patients with coronary artery disease and an ageand sex-matched control group; however, anti-CMV IgG antibody titer was higher in patients with coronary artery disease (52% of the patients had .1:800 anti CMV IgG titer, whereas only 16% of the control group had high antibody titer).” I believe that antibody titer can differentiate between patients with coronary artery disease (high IgG titer, #1:800) and people who are seropositive ($1:400) for anti-CMV IgG antibodies. The mechanism of action could be direct activation of smooth muscle cells in the vessel wall and/or chronic inflammation that triggers and activates a cascade of events that eventually cause atherosclerosis. It would be interesting to measure inflammatory proteins and other markers of inflammation (such as vascular cell adhesion molecules) periodically (every 6 months) in people with high titers of anti-CMV IgG antibodies and to correlate these measurements with clinical events and outcome. For example, our study demonstrated that a high anti-CMV IgG antibody titer is associated with coronary artery disease and could predict restenosis after coronary balloon angioplasty.2

Are the high levels of cytomegalovirus antibodies a determinant in the development of coronary artery disease?

In several epidemiological studies, it was suggested that a high titer of cytomegalo-virus (CMV) antibody meant CMV reactivation, and that this condition was a determinant of coronary artery disease (CAD). The purpose of this study was to investigate both the prevalence of the CMV infections in our study population and whether high CMV sero-positivity is a determinant of CAD. Blood samples from...

S TUDY OF CYTOMEGAL OVIRUS ANTIB ODY IN ATHEROSCLEROTIC PATIENTS IN TABRIZ

To evaluate the correlation of cytomegalovirus (CMV) and coronary artery disease (CAD), one-hundred patients with CAD and the same number of healthy individuals as a control group were randomly selected and studied for anti-CMV using ELISA method. Ninety percent of patients and 77 percent of the control group were anti-CMV positive and statistical analysis showed a significant correlation ...

Seroprevalence of cytomegalovirus in patients with and without coronary artery diseases at Madani Heart Center, Iran.

Inflammation plays a major role in coronary artery disease (CAD). Currently, it is unclear, whether Cytomegalovirus (CMV) infection is associated with the risk of the atherosclerosis. The aim of this study was to determine the prevalence of anti- CMV antibodies in CAD and non CAD patients undergoing artery bypass surgery. Sera from 157 patients who underwent coronary angiography were tested for...

Prior infection with cytomegalovirus is not a major risk factor for angiographically demonstrated coronary artery atherosclerosis.

To determine if cytomegalovirus (CMV) infection is a risk factor for primary coronary artery disease (CAD), the association between CMV infection and CAD (>50% blockage in any coronary artery) was investigated in nearly 900 successive nontransplant patients undergoing coronary angiography. By use of logistic regression, older age (P <.001), white race (P <.001), gender (P <.001), hypercholester...
